The Halton Catholic District School Board supports collaborative relationships that are consistent with our Vision, Mission, and Values, as well as with our existing policies and procedures in a Catholic context. Service agreements are recognized as mutually beneficial, and serve as supportive arrangements between a school and an external mental health, physical health or social service agency, professional or paraprofessional. The intent of these collaborative relationships is to enhance and expand opportunities for student success and well-being.

 

Collaborative relationships are a coordinated effort between external agency personnel and the school Principal, as well as with student service professionals, and other relevant school staff. The agreement should be designed to meet an identified need within a school community.

Professional Services Request

 

An external agency that wishes to establish a collaborative relationship agreement with the Halton Catholic District School Board must do so by completing the following: (1) A Professional Services Agreement Request Form and (2) All required documents on the Required Documentation Checklist.
